Add additional fields for instructors on class petitions page in my.harvard 

Project Description: Faculty would like academic level (sophomore, junior, etc) added to the petitions page in my.harvard to assist with the approval/deny process for limited enrollment courses. Add these criteria to the petition list view on several different pages in Teaching/Advising in my.harvard and to relevant filters. Change is needed to support different registration periods for incoming and returning students for fall.
